Under the guidelines of the Iowa Department of Public Health and the CDC, Community Health Partners works to ensure the health and safety of the community by tracking communicable diseases in Sioux County. Community Health Partners, as the local public health provider, is responsible for communicable disease reporting, monitoring, surveillance, and investigation.

Public Health nurses are available to answer questions you may have about a communicable disease and how it is transmitted and treated.
