Home Care & Hospice Services

Home Care

Medicare-certified in-home health services include skilled nursing, personal care, physical, speech and occupational therapy services. These services are provided under a physician authorized plan of care.

All persons are eligible for service regardless of age, sex, race or socioeconomic status. Some in-home health services are covered by Medicare and/or private insurance. Non-skilled services may not require a physician plan of care. These services may also not be covered by insurance or entitlement programs. A sliding fee scale may be used to determine the cost of these services.

Hospice

Medicare-certified Hospice care is a program of comprehensive services provided by a multi-disciplinary team to an individual (and his/her family) with a diagnosis of a life limiting illness. Services are provided to the Hospice client in his/her home or in a home like setting (hospice room in a hospital or long-term care setting). The overall goal of care in Hospice is palliation (symptom management, comfort, pain control).

Chore Services

Chore services provide assistance to persons having difficulty with one or more essential activities of daily living: heavy housework, yardwork, sidewalk maintenance, home repair, etc. Chore service in Sioux County is available using a voucher system. Sioux County residents, age 60 and older who are low (< $1737 per month) to moderate (> $1692 per month and < $2394 per month) income, may request compensation to assist them in paying for the cost of home repairs, heavy housework, yardwork, snow removal, etc. Recipients request this reimbursement in writing. A fee scale will be used to establish the contribution level.
